Rheumatology billing for practices and groups: visits for arthritis and autoimmune disease, infused biologics and their biosimilars, joint injections and musculoskeletal ultrasound, and bone density testing, worked inside the software you already use.
Rheumatology billing centers on the infusion chair: getting every visit, infusion and injection paid in full. Biologics are billed by the unit with the administration code the payer accepts, the right product is matched to each plan’s preferred list, joint injections are coded with the guidance used, and denied infusions are appealed with the records behind them.

Much of a rheumatology practice’s revenue runs through its infusion chairs. Biologics such as infliximab and rituximab cost thousands of dollars per dose, plans want the drug they prefer, often a biosimilar, approved before the first infusion, and the administration code depends on the drug and the payer. Visits for long-term disease management and joint injections fill the rest of the day.
The drug, the plan and the infusion record decide the claim. That is why we check the product, units, authorization and administration code on every infusion before the claim goes out.
Rheumatology practices bill visits, infusions and injections for the same patients month after month. Each one has its own failure point. Here is where each breaks, and what we do about it.

Each biologic has a HCPCS code with a fixed unit size, and the claim pays for units, not vials. Infliximab (J1745) and its biosimilars (Q5103, Q5104 and Q5121) are billed per 10 mg, so a 500 mg dose is 50 units. Rituximab (J9312) is per 10 mg, tocilizumab (J3262) per 1 mg and abatacept (J0129) per 10 mg. Single-dose vials need JZ when nothing was discarded or a JW line for the waste.
The administration code is the other half. CPT places highly complex biologics, such as many monoclonal antibodies, in the chemotherapy administration family (96413 and 96415), and several Medicare contractors publish lists of the drugs they accept there; other plans pay only the standard infusion codes (96365 and 96366). Plans also steer patients to a preferred product, often a biosimilar, and deny the others.
What we do: before an infusion is scheduled, we confirm the plan’s preferred product and the authorization. Before the claim goes out, we check the weight-based dose against the units billed, the waste modifier and the administration code that payer accepts for that drug.
Rheumatology claims combine high-cost drugs with procedure and imaging codes. These are the ones that decide most claims, with what payers check on each.
| Code | What it covers | What payers look for |
|---|---|---|
| 99202 to 99215 | Office visits (E/M) | The level matches the decision making or time documented; modifier 25 with a same-day procedure only when the visit is separate. |
| G2211 | Add-on for ongoing care | Medicare, for patients whose serious or complex condition you manage over time. |
| J1745Q5103Q5104Q5121 | Infliximab and its biosimilars, per 10 mg | The plan's preferred product, units from the dose, and JZ or JW. |
| J9312 | Rituximab, per 10 mg | Authorization and the diagnosis on the plan's covered list. |
| J3262J0129 | Tocilizumab (per 1 mg) and abatacept (per 10 mg) | Weight-based dosing converted to units. |

A visit significant and separate from a same-day injection or infusion. Modifier 25 explained
Required by Medicare on single-dose drugs when the whole dose was given.
The unused part of a single-dose vial, billed on its own line.
The joint's side, for plans that want each side on its own line.
The same injection in both joints, where the payer accepts 50.

It is not for every practice. If you are a hospital-employed group whose billing the hospital handles, you do not need us. And if your in-house team keeps denials low and receivables current, keep them, and consider a periodic billing audit instead.
We look at your clean-claim rate, your denials by payer and reason, biologic units and administration codes, preferred products and authorizations, and your aging receivables, and tell you what we find, whether or not you go further with us.
We get access to your practice management system and clearinghouse, load each payer's authorization and bundling rules and your fee schedules, list active infusion patients and their authorizations, and agree how escalations work. Most practices are live within one to two weeks.
Visits, infusions and injections are coded and claims submitted within 24 hours. Rejections are corrected the same day, remittances posted as they arrive, products, units and administration codes checked, and denials worked to their cause, all inside your system.
A report you can read in ten minutes: what was billed and collected by payer, site and service line, what was denied and why, authorizations pending, and what is aging.
You do not change systems to work with Summit Billing Solutions. We work in the practice management and EHR systems below, and through the clearinghouse you already use. If your EHR is not shown, ask; the list grows as we onboard practices.
Before a rheumatology claim goes out, we check the product against the plan's preferred list, the units against the dose, the waste modifier and the administration code that payer accepts.
You know who is working your account, and you hear from them every week. A familiar face, not a ticket queue.
If we are not earning the relationship, you can leave. That keeps us honest.

When biologic infusions make up much of a practice’s revenue, a single miscounted unit or missing authorization can cost more than a month of visits. A fee tied to collections keeps us focused on getting those high-value claims paid in full.
Summit Billing Solutions charges between 3% and 9% of monthly collections. Your rate depends on infusion volume, how much drug you buy and bill, authorization volume and how clean the receivables are, which the free account review shows.

We bill for more than 40 specialties, for practices in all 50 states, from Salem, New Hampshire. Each one has its own codes, modifiers and payer rules.
A free account review looks at your clean-claim rate, your denials by payer and reason, biologic units, preferred products, administration codes and authorizations, and your aging receivables, and tells you what we would change. No cost, no obligation, and no pressure to continue. What the review covers, step by step, is on the free account review page.
