What sleep medicine billing includes

Sleep medicine billing gets studies, readings and follow-up visits paid, and keeps patients’ PAP coverage intact. Studies are coded by type and split between the lab and the reader, the documentation suppliers need for PAP coverage is in the chart on time, and patient balances are billed only after the plans have paid.

of those CPAP errors were insufficient documentation, and another 18.3% failed medical necessity

A sleep practice’s documentation decides whether a patient’s therapy is paid, even when someone else bills the device. Medicare covers PAP for an initial 12 weeks, and continued coverage depends on a follow-up visit in a set window and objective proof the patient is using the machine. Meanwhile the studies themselves are billed by type, device and site, often split between a lab and the physician who reads them.

The study type and the follow-up note decide the claim. That is why we check the study codes, the components and the timing of every follow-up visit before claims go out.

Sleep studies, PAP therapy and follow-up

Sleep practices bill studies, interpretations and visits, and support device claims billed by suppliers. Each step has its own rules. Here are the failure points, and our fix for each.

Keeping PAP covered: the 12-week window and the adherence rule

Medicare covers a PAP device for obstructive sleep apnea for an initial 12-week period. To start, the patient needs a face-to-face evaluation and a covered sleep test showing the required number of breathing events per hour. To keep coverage after the first 12 weeks, the treating practitioner must see the patient again no sooner than the 31st day and no later than the 91st day after therapy starts, and document that symptoms have improved.

The note also needs objective adherence: use of the device for at least four hours a night on 70% of nights during a consecutive 30-day period in the first three months. If the patient misses that window, the supplier cannot simply continue. The device is billed by the supplier, but the coverage rests on the sleep practice’s notes.

What we do: when PAP therapy starts, we note the start date and the follow-up window, check that the visit is scheduled inside it, and confirm the follow-up note records the usage data and symptom change suppliers and payers look for.

Before you bill

  1. Start date: the date PAP therapy began
  2. Window: day 31 to day 91 for the follow-up visit
  3. Usage: a download or report covering a 30-day stretch in the first three months
  4. Symptoms: how sleepiness and other symptoms have changed

What goes on the claim

  1. Follow-up visit: the office or telehealth visit, coded from the note
  2. Adherence: at least four hours a night on 70% of nights in a consecutive 30 days
  3. Supplier: notes sent to the device supplier when requested
  4. Management: 94660 only when separate from the visit

Sleep study and PAP management codes

Sleep claims depend on the study type, the device and who owns the equipment. These are the codes we see most, with what payers check on each.

Is outsourced sleep medicine billing right for you?

It is not for every practice. If you are a hospital-employed group whose billing the hospital handles, you do not need us. And if your in-house team keeps denials low and receivables current, keep them, and consider a periodic billing audit instead.

How we take over your billing

  1. Free account review

We look at your clean-claim rate, your denials by payer and reason, sleep study coding and components, PAP follow-up timing, authorizations, and your aging receivables, and tell you what we find, whether or not you go further with us.

  2. Onboarding

We get access to your practice management system and clearinghouse, load each payer's authorization and bundling rules and your fee schedules, list each lab you read for and patients starting PAP therapy, and agree how escalations work. Most practices are live within one to two weeks.

  3. Daily billing

Studies, readings and visits are coded and claims submitted within 24 hours. Rejections are corrected the same day, remittances posted as they arrive, study types, components and follow-up windows checked, and denials worked to their cause, all inside your system.

  4. Monthly reporting

A report you can read in ten minutes: what was billed and collected by payer, site and service line, what was denied and why, authorizations pending, and what is aging.

We work with your software

You do not change systems to work with Summit Billing Solutions. We work in the practice management and EHR systems below, and through the clearinghouse you already use. If your EHR is not shown, ask; the list grows as we onboard practices.

Sleep medicine billing pricing

Sleep practices earn from studies, readings and follow-up visits, and a missed follow-up can cost the patient their therapy coverage. A fee based on collections keeps us working on the whole path, not just the first claim.

Summit Billing Solutions charges between 3% and 9% of monthly collections. Your rate depends on study volume, how many labs you read for, the share of home versus in-lab testing and the state of the receivables, which we see in the free account review.

Other specialties we bill for

We bill for more than 40 specialties, for practices in all 50 states, from Salem, New Hampshire. Each one has its own codes, modifiers and payer rules.

Sleep medicine billing questions

Most billing companies charge a percentage of collections, and a few charge a flat monthly or per-claim fee. Summit Billing Solutions charges between 3% and 9% of monthly collections. For a sleep medicine practice, the rate depends on study volume, how many labs you read for, the share of home versus in-lab testing and the state of your receivables, and we quote your exact rate after the free account review.

Find the sleep claims you are missing

A free account review looks at your clean-claim rate, your denials by payer and reason, sleep study coding and components, PAP follow-up timing and authorizations, and your aging receivables, and tells you what we would change. No cost, no obligation, and no pressure to continue. What the review covers, step by step, is on the free account review page.
